SEP Case Manager The Upper Arkansas Council of Governments, Area Agency on Aging is seeking a dynamic, goal orientated individual to join our team as Case Manager for the Single Entry Point (SEP) program serving Lake, Chaffee & Park Counties.
Experience in HCBS (Home & Community Based Services) Long Term Care Medicaid Home Care Allowance & Adult Foster Care.
Must be a proven problem solver, have excellent verbal & written skills.
Must have bachelors degree in human services or other related field.
Experience as a caseworker/manager w/a long term client population may be substituted on a year for year basis.
Must have a strong desire to work directly with at risk populations.
Must successfully complete background check.
Must be proficient in Microsoft Windows Travel required.
Must possess a valid Colorado driver's license & vehicle insurance.
40 hrs week.
Salary:
$27,560 to 43, 659 Location:

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
